Back when I still wore long, winter pajamas (as a kid, teen, etc.), I don't ever remember wearing underwear underneath them. Maybe if it was REALLY cold that night, or if I was really tired and didn't want to bother removing them, but for the most part, no.

Presently, "pajamas" are a pair of boxer shorts, and a T-shirt by the side of the bed if my son wakes up in the middle of the night, since I doubt he really wants to see my flabby stomach. So I guess that technically, since I'm wearing boxers, my pajamas and underwear are now one and the same, and no underwear underneath. That just sounds very weird (yet obviously making perfect sense for network television, which makes me wonder if Ozzie and Harriet or Jim and Margaret Anderson just had their pajamas on underneath their clothes at all time, thus ensuring no skin showed whatsoever if they ever had to change like that on-camera).
