There is no hint that Johnny Sack's family struggles with the way he makes a living, like each member of the Soprano family does. Johnny Sack's family loves him, but they seem vaguely spoiled too. The fact that the wife dregs up the way Johnny wouldn't quit smoking, even though she lost 27 pounds, seems pertinent; of all the monstrous things Johnny did in his life, for her to concentrate on that common vice as a point of marital contention (at that time in the past, before she knew he had cancer) seems pretty silly.
